Points and Pearls Excerpt

  • Risk factors for maternal stroke include the physiological changes of pregnancy and the potential for pathophysiological sequelae from these changes, as well as non–pregnancy-related stroke risk factors. (See Table 1.)
  • The risk of maternal stroke peaks in the peripartum and immediate postpartum periods, when patients have likely been discharged home after delivery.
  • Hypertensive disorders of pregnancy (ie, pre-eclampsia, eclampsia, and HELLP syndrome) should be highest on the differential for pregnant and postpartum patients who present to the ED with a headache. These are “can’t-miss” diagnoses. (See Table 2.)
  • The NIHSS or other standardized stroke scale should be used in the assessment of pregnant and postpartum patients with stroke. The score will help guide treatment decisions and risk/benefit considerations for intervention.
