  • Fever in the returning traveler requires a structured, systematic approach because early presentations are often nonspecific.
  • Diseases that are life-threatening, prevalent in travelers, and consistent with travel timing and incubation period should be prioritized. (See Table 1 and Figure 5.)
  • A focused travel history, including destination, timing, activities, and vaccination and prophylaxis history, is the single most important component of evaluation and should guide targeted diagnostic workup. Failure to obtain a travel history is a leading contributor to misdiagnosis of malaria and other serious tropical infections.
