Magnesium Sulfate Use In Vaso-Occlusive Crisis

During VOC, patients have depleted nitric oxide stores and raised levels of vasoactive factors such as endothelin and thromboxane, resulting in vasoconstriction. In magnesium-deficiency states, inflammatory cytokines, interleukins, and TNF-alpha are increased. Dehydrated erythrocytes are also at risk for sickling. Magnesium sulfate supplementation improves erythrocyte hydration and modulates endothelial dysfunction by increasing endothelial nitric oxide, causing a vasodilatory effect.121
