Hypertensive urgency without evidence of endorgan damage can be monitored on a telemetry unit or an intermediate step-down unit. Patients with a hypertensive emergency may require ICU admission, but, depending on the comfort level of the hospital, they may be admitted to a telemetry unit or an intermediate-care unit. Although there are no specific consensus guidelines for telemetry monitoring with extreme hypertension, it would be useful to monitor these patients for early acute coronary syndromes or unstable angina and arrhythmias as well as during administration of intravenous antihypertensives.90
