Charting & Reimbursement: What You Need To Know

Documentation of sickle cell crisis (SCC) sounds easy. Understandably, we’re more focused on fixing rather than documenting, but too often, the doctor’s assessment only states “sickle cell crisis.” While you often provide antibiotics, opioids, hydration, transfusions, and other services without documentation of secondary issues, the hospital cannot claim credit for these services and your SCC patients appear less ill than your peers’. For ideal documentation in SCD admissions, note the following: (1) the trigger of the crisis, (2) any end-organ dysfunction, (3) complications of the treatment, and (4) underlying chronic comorbid conditions. Proper capture of severity of illness depends on noting the secondary issues.
