Disposition And Transitions Of Care

One of the greatest challenges in the management of a SCD patient for the hospitalist is the disposition. When determining diagnostic and management goals of patients with SCD, hospitalists should play the critical role of interpreter, finding a balance between initial history, physical examination, prior hospitalizations, diagnostic studies, and history of presenting illness, if applicable. It is then the hospitalist’s role to triage, based on these characteristics in various clinical presentations, including inpatient admissions. At the time of discharge, physicians should also assess certain healthcare maintenance measures. These measures serve to not only guide transition of care but also as SCD complication preventative measures.
