Medication adherence, specifically with hydroxyurea, should be reinforced to the patient in the emergency department, during hospitalization, and prior to discharge. The benefits of hydroxyurea therapy include decreased rates of pain crisis and acute chest syndrome episodes, and the need of hydroxyurea treatment should be explained to the patient. If there is inappropriate follow-up, a lack of adherence, or if the patient is transfused, hydroxyurea should be stopped. With an understanding of clear benefits and consistent adherence, adverse effects (such as myelotoxicity) and the importance of deferring pregnancy should be explained as well.
