This does not lessen clot burden initially, but it does prevent any extension of a clot and allows the body’s own fibrinolytic processes to dissolve an existing clot. In hemodynamically stable patients, low-molecular-weight heparin or unfractionated heparin alone are effective and remain the standard of care for initial anticoagulation.78,79
