Primary injury from burn insult compromises mechanical immunity and provides ready access to lymph or blood for bacteria that can proliferate in the milieu inherent to the injured, necrotic tissue. As such, burn-injured patients should be considered to be in an immunocompromised state,100-102 leading some to suggest a role for systemic prophylaxis. The current standard of care is to defer their use,67,103 though no high-quality evidence has been offered to settle the issue definitively.
