Over the past decade, endovascular therapy has revolutionized the management of acute ischemic stroke. Moreover, it has fundamentally altered the way in which emergency clinicians may approach the patient presenting with acute stroke. Traditionally, the only modality available to acute ischemic stroke patients has been IV rt-PA. Unfortunately, < 1% of acute ischemic stroke patients in the United States receive rt-PA, primarily because of a delay in presentation for treatment.63
